Review factual details, injury observations and confidentiality before submitting a safety report.
Choose the report type and severity. Record when it happened, the location and a clear title.
Describe what happened or could happen and any immediate action taken. Select an affected employee only when appropriate.
For an incident or accident involving an injury, select the injury option and record the available body areas and observations. Record observations rather than a diagnosis; use Not sure when details are unknown.
Check whether the report needs the confidential restriction. This restricts manager access; authorised HR and people directly involved may still see the report under workspace rules.
On Android, choose Review report. Check the facts and use Edit details if something needs correcting, then choose Submit report.
Reopen the reported record to check its saved status. Assessment, corrective actions, verification and closure follow separately.
The report enters the workspace’s safety workflow; assessment, actions and closure are separate management steps.
For immediate danger, follow your workplace emergency procedure first. If submission appears uncertain, check the report list before submitting again. A confidential restriction does not make the report invisible to authorised HR.
